Why are we reluctant to say sorry?

I unfailingly hesitate when it comes to apologising. It would lead me to wonder whether it is my ego coming in the way or whether it is due to shame.

Doctor Aniello Ianuzzi, a GP, author and philosopher from Coonabarabran believes it is hard to apologise because    "in many ways the black and white between right and wrong has become very much blurred. Therefore people don't generally believe they're wrong anymore."

His explanation however leaves one question hanging, why are we still reluctant to apologise even when we know ourselves to be wrong?
